Pemberdayaan Pasien Diabetes Melitus melalui Edukasi Lima Pilar Diabetes untuk Meningkatkan Kepatuhan Manajemen Mandiri di Rumah Sakit Pertamina Jaya

Abstract


Diabetes mellitus is a chronic disease requiring active patient involvement in daily disease management. A patient's ability to engage in self-management plays a crucial role in maintaining health and preventing complications. This community service activity aimed to enhance the knowledge of diabetes mellitus patients regarding disease management through education on the "Five Pillars of Diabetes" and practical sessions on diabetic foot exercises. The activity was conducted on September 1, 2026, at the Internal Medicine Clinic of Pertamina Jaya Hospital, involving 20 patients with diabetes mellitus. The methods employed included health education, interactive discussions, demonstrations and practice of diabetic foot exercises, knowledge evaluation using pre- and post-tests, and blood glucose screening. Educational materials covered dietary management, physical activity, pharmacological therapy, blood glucose monitoring, and complication prevention. Evaluation results showed an increase in participants' knowledge scores from 75 in the pre-test to 98 in the post-test—a rise of 23 points, or approximately 30.7%, from the baseline. Blood glucose screening revealed an average level of around 200 mg/dL. Diabetic foot exercises were conducted as part of physical activity and participant empowerment, equipping them with skills that could be applied independently. The activity demonstrated that combining education with practical application improves participants' knowledge regarding diabetes management. Continuous education and appropriate physical activity practices need to be developed to support self-management among patients with diabetes mellitus
